QUALIFYING FOR MEDICAL CANNABIS IN OHIO

To qualify for a patient registry identification card, a qualifying patient must complete the following:

Visit a certified physician who can confirm that you have one of the medical conditions that qualify for medical marijuana and have the physician create your profile in the Patient & Caregiver Registry.
Confirm and complete your registration for the program through the Patient & Caregiver Registry.

PATIENT QUALIFYING CONDITIONS FOR OHIO

Health care providers registered with the  Ohio  Department of Health can certify OH residents diagnosed with these conditions:

Certified physicians have access to the Patient Registry and will submit their recommendations for a patient to receive medical marijuana directly to the Patient Registry.
The physician will need the patient’s valid Ohio driver's license, a valid Ohio identification card issued by the Ohio bureau of motor vehicles (BMV), or a valid United States passport. 
A registered caregiver may possess and administer medical marijuana to patients with whom the caregiver's registration is associated. The certified physician will need similar  identification from a caregiver to establish their profile in the Patient Registry.
A certified physician can recommend up to a 90-day supply of medical marijuana with three refills (totaling up to 360-day supply if appropriate for the patient).

STATE APPLICATION COSTS

Once a recommending physician has entered a patient or caregiver into the registry, the patient/caregiver will receive an email prompting them to login to their profile in the Patient Registry. In the registry, complete your application and pay the fee.

Ohio Department of Health Application Fee – $50 for patients and $25 for caregivers. Patients who qualify for indigent or veteran status may be eligible for fee reductions. To do so, a physician with a certificate to recommend medical marijuana must register the patient in the Patient & Caregiver Registry and indicate that the patient is registering with special status. Then, the patient must then submit documentation to the State of Ohio Board of Pharmacy to qualify for their special status.

Indigent Status
Those applying for indigent status must provide a copy of a letter or other documentation from one of the following program types:
A. Enrolled in the federal "Social Security Disability Income" (SSDI) or the "Supplemental Security
Income" (SSI) disability programs. Provide documentation showing:
1. The identity of the patient; and
2. The amount of monthly Social Security Disability Income or Supplemental Security Income
benefits the individual will receive during the calendar year they register with the Ohio Medical
Marijuana Control Program.
or
B. Enrolled in a State Retirement System and receiving monthly disability benefits. Examples: a state
public employees retirement system, a state teacher retirement system, and a police or fire
retirement system. Provide documentation showing:
1. Identity of the patient; and
2. Amount of monthly disability benefits to be received by the prospective patient during the current year of the registration submission. 

Veteran Status
Those applying for veteran status must provide a copy of any of the following documents:
1. Department of Defense Identification Card (active, retired, temporary disability retirement
list);
2. DD214, DD215, or National Guard Bureau military discharge certificate indicating disposition of discharge;
3. Report of separation from the National Archives National Personnel Records Center in St. Louis, Mo.; or
4. Veterans Identification Card from the Department of Veterans Affairs.

STATE APPLICATION PROCESS

1. Talk with your Health Care Provider
If your health care provider decides that medical marijuana is right for your qualifying  condition, you will get a signed certification form. You need this form to apply for a

2. Apply for Registry ID Card Online
 Once a recommending physician has entered a patient or caregiver into the registry, the patient/caregiver will receive an email prompting them to login to their profile in the Patient Registry. In the registry, complete your application and pay the fee.

POCESSION AND CULTIVATION OF MEDICAL CANNABIS IN OHIO

Ohio has a tiered possession limit, allowing a certain amount of THC-infused cannabis products to a patient depending on the condition they have. A tier I medical cannabis cardholder can possess up to 8 ounces of THC that tests at 23% or less; and a tier II medical cannabis cardholder can have up to 5.3 ounces of THC that test above 23%, but not higher than 35%.

Certain types of products in Ohio are only allowed to have so much THC. While patches, lotions, creams, and additional topics can not exceed 26-55 grams of THC, cannabis oil, tincture, capsules, and all other edibles can not exceed 9.9 grams of THC. Up to 53.1 grams of THC is allowable specifically for oil that is to be vaporized.

CAREGIVERS IN OHIO

Ohio’s caregiver laws are straightforward, beginning with the patient assistant being at least 21 years old and able to prove their Ohio residency with a valid driver’s license or state identification card. The caregiver, who must be listed on the patient certification, must also pay a $50 annual card registration fee. Ohio caregivers can only care for two patients at a time per law.

Registration of a patient or caregiver will be valid from the date of issue and will expire one year later, the last day of the month it was given. The patient’s registration expires after six months if the patient is diagnosed as terminally ill.
