Ohio Provisions Carroll Registration
Below are the three simple steps you need to make to register for your Ohio medical marijuana card.

Find A CTR Doctor
Find and schedule a visit with a (CTR) certified to recommend medical marijuana doctor.
- The physician will need the patient’s valid Ohio driver's license, a valid Ohio identification card issued by the Ohio bureau of motor vehicles (BMV), or a valid United States passport.
- Certified doctors will have access to submit patient recommendations to the Patient Registry.
- Find a CTR doctor in your area.
Visit Ohio Provisions Carroll, Ohio Dispensary
Once you have received your Patient or Caregiver Registry Card, you will be able to purchase from any of our Provisions Medicinal dispensaries or any other dispensary licensed by the State of Ohio Board of Pharmacy.
Additional hours
We are open Monday–Saturday from 10:00am-7:00pm and Sunday from 11:00am-5:00pm. We have added
additional hours from 9:00-10:00 am Monday–Saturday, and 10:00-11:00 am on Sunday for immunocompromised patients and those who are 60+.
